Beginning of Year Reports

The Beginning of the School Year can be a busy time and PTA is no exception.  There are a few forms and reports that each PTA Unit is required to turn into Utah PTA via Givebacks or their Council/Region. 

The Beginning of the Year Reports include:

  • An Approved Budget with Minutes from the General Membership Meeting signed by the President and Secretary
  • Signed by all Board Members, the Conflict of Interest Policy Form
  • Signed by all Board Members, the Basic Fiscal Management Form
  • Copy of Proof of Insurance
  • Enter all Board members into Givebacks or turn in list to council/region treasurer
  • At least 2 board members attend training and reported on Report on Training Form
